WNY Rugby Academy Event Waiver and Release of Liability
Please read carefully. By checking the box when you register, you agree to these terms for the event you are registering for and any WNY Rugby Academy event you register for later while this waiver is posted.
Organizer: WNY Rugby Academy ("WNY Rugby Academy," "we"). Our events are not sanctioned by USA Rugby unless we state otherwise.
1. WHO IS SIGNING
I am 18 or older and am signing for myself. If under 18, a legal parent and/or guardian is signing on my behalf.
2. I UNDERSTAND THE RISKS
Rugby is a contact sport. Training, drills, tackling, running, testing and games involve risks, including serious injury, concussion, paralysis and, in rare cases, death. These risks exist even with proper coaching, equipment and rules. I understand them and choose to take part voluntarily.
3. CONCUSSIONS
I understand the risks of head injuries. If I or a coach suspect I have a concussion, I will stop playing and accept removal from activity. I will seek medical care before returning.
4. MY HEALTH
I confirm I am physically fit for athletic activity and have no medical condition that makes it unsafe for me to take part. I will tell a coach right away if I feel unwell or injured.
5. RELEASE
To the fullest extent allowed by law, I release and agree not to sue WNY Rugby Academy, its directors, officers, coaches, volunteers and agents, and the host venue, for any injury, loss or damage arising from my participation, including from ordinary negligence.
6. EMERGENCY CARE I authorize WNY Rugby Academy to arrange emergency medical care for me and to share my emergency contact's information with medical providers. I am responsible for any costs.
7. RULES I agree to follow the Participant Code of Conduct, coaches' instructions and the venue's rules. I may be removed from the event if I do not.
8. INSURANCE I understand I may need to rely on my own health insurance. WNY Rugby Academy does not provide medical insurance for participants.
9. PHOTOS AND VIDEO I give WNY Rugby Academy permission to use photos and video of me from events for promotion or social media.
10. GENERAL New York law governs this waiver. If any part is found unenforceable, the rest still applies. Checking the box at registration is my electronic signature. I have read this waiver, understand it, and agree to it.
11. REFUNDS AND CANCELLATIONS Registration fees are non-refundable and non-transferable. WNY Rugby Academy may, at its sole discretion, offer a refund, credit or transfer in individual cases. If we cancel or reschedule an event, we will tell registered participants and may offer a refund, credit or transfer to a future event. We are not responsible for travel, lodging or other costs you incur. We may also cancel or change an event for weather, safety, venue availability or other reasons.
